We charge by the 65 character line, which is universally considered the industry standard for medical/surgical/medico-legal transcription. We add up all the characters (including spaces) that we actually type and then divide by 65 i.e formatted fixed template characters such as headers, footers, pre-defined headings, signature blocks etc. are excluded from the character/line count.

MS Word also provides a fairly accurate means of determining the character/line count per document. If you highlight the physically typed sections of a particular document and then select the Word Count option under Review, noting down the 'Characters (with spaces)' number and then divide this number by 65. This will give you the number of 65 character lines for the document.
